Privacy & Medical Records

Student Health protects your privacy and keeps your medical information confidential. Records are not shared with parents, friends, faculty, employers, or others without your signed authorization.

Information may only be released:

• In a medical emergency (e.g., if you’re incapacitated)
• If required by law or subpoena

Medical Record Releases

• Releases are completed case-by-case (blanket releases are discouraged).
• A new form is required each time records are sent.
• A medical power of attorney typically cannot access records.

Referrals & Record Requests
Our staff can help you:

• Send or request medical records
• Coordinate referrals to outside providers
• Verify insurance participation
• Schedule initial appointments
• Send relevant medical information

First Visit
All new patients must:

• Provide consent for treatment
• Acknowledge receipt of the Notice of Privacy Practices.
