Immunization Requirements

Please refrain from leaving multiple emails and voicemails to our department during our peak times. Doing so will only delay our response time. 

COMPLIANCE WITH NC IMMUNIZATION LAW

North Carolina state law requires anyone entering college to present a certificate of immunization that documents their compliance with all required immunizations. The Immunization Department gladly accepts these records for review and serves to assist UNC Charlotte students with questions about becoming compliant with this law. Once complete records are received, the Immunization Department documents this compliance for the University Office of the Registrar.  Records are due at the time of your admission to the University and prior to your arrival to New Student Orientation. The statute applies to all students except students residing off-campus and registering for any combination of:

  • Off-campus courses (distance education program)
  • Evening courses (classes beginning after 5 p.m.)
  • Weekend courses
  • No more than four day (before 5 pm) credit hours in on-campus courses 
    • If taking 5 or more credit hours before 5 pm immunization records are to be submitted

*Enrollment in only online classes does not qualify you as a distance  
education student.

*While your State or Country of origin may have different immunization requirements, you must comply with North Carolina laws and UNC Charlotte requirements*

Immunization records are NOT automatically transferred from your high school or previous college or university.

All required vaccines are available at Student Health. Charges associated with each vaccine.

Acceptable Forms of Immunization Records

If you already have a copy of your immunization records that is stamped or signed by the administering physician/health agency and includes dates on which immunizations were administered, you can use this as proof of your immunizations. Please include your name, student identification number, and date of birth on all pages uploaded.

You may print a copy of the Immunization Form and submit it to your physician to complete and sign. You may download this form from the Form Library. If you would prefer you may call to have this form mailed to you.

For transfers, if you have attended another college or university that also required immunizations, contact the former university's health center to request that a copy of your records be sent to you (preferable) or to UNC Charlotte Student Health. Your immunization records do not transfer automatically.

*Please note: If you have received any vaccines in North Carolina, they may automatically be pulled from the North Carolina Immunization Registry (NCIR) and you may see them in the health portal. Prior to entering vaccince dates, please review any previously entered dates. You do not need to re-enter vaccine dates if they are already listed below.

ALWAYS KEEP A COPY OF ANY RECORDS SUBMITTED FOR REVIEW; IT IS IMPORTANT THAT YOU MAINTAIN YOUR OWN MEDICAL RECORD OF VACCINES RECEIVED
